Heather Mason wakes up from a terrifying amusement park dream only to find her local shopping mall warped into a grotesque, flesh-lined nightmare. After a strange meeting with a private detective named Douglas Cartland and a cult priestess named Claudia Wolf, Heather must fight her way through horrific monsters to reach the subway and escape.
Trying to find a way home, Heather navigates the maze-like, abandoned subway tunnels and an eerie under-construction office building. The boundaries of reality continue to splinter as she encounters a strange woman named Vincent and uncovers increasingly disturbing clues about her own origin.
Following a devastating tragedy at her apartment, Heather travels to the fog-swept town of Silent Hill alongside Douglas. Looking for answers, she searches Brookhaven Hospital, only to unleash a terrifying shift into a shifting, blood-soaked Otherworld filled with demonic nurses and repressed trauma.
Heather heads to the Lakeside Amusement Park to finally confront Claudia. Navigating the dark, surreal carnival attractions from her opening nightmare, she faces a twisted, bloody reflection of herself and uncovers the full weight of her past life's identity.
Descending into the massive cult chapel hidden beneath the amusement park, Heather uncovers the final plans of the Order. Confronting both Vincent and Claudia in a desperate showdown, she must destroy the grotesque, newly birthed deity once and for all to break the cycle of suffering.
